Here is a Sample Apology letter from Doctor to Patient. You should  modify this apology letter according to your need and situation.

Respected Sir/Madam,

I am quite aware that you were not satisfied on the service which our medical clinic is offering to you.

But you need to accept the fact that this clinic having located in a remote village like ours is providing a more than sufficient, best in class, medical service for general medical assistance. The equipments and laboratories of this medical clinic is restricted to a particular limit based on the people and the associated diseases corresponding to our village’s general medical problems.

The disease which you have attained seems to be a very specific disease which our laboratory technicians could not identify exactly using the collected blood and urine samples of yours. We have also sent a portion of those samples to the nearest City hospital for which the result is pending. We believe that it might take more time to get those results.

Having not even identified the disease, the medication which we prescribe to you would lead you with adverse health effects such that it might worsen your health even towards death. So the clinic has decided to stop all its medications except some little doses of general medicines and recommend you to discharge from this clinic to a well equipped Medical Institution located in a Metro City.

With the hope that you could understand my situation, as well as my care towards your health, I sincerely apologize for the inconveniences you have faced with our clinic. I would be very happy if god restores your health back to normal due to proper medical assistance.
